Add Patients

To Add Patients to metaclinic, go to Patients>+ New Patient 

This takes you to the Patient Create page to enter all patient information, such as:

• Patient Details
  • First Name
  • Middle Name
  • Last Name
  • Prefix
  • Suffix
  • SSN
  • Sex

• Address
  • Primary or Alternate Address
  • Address Type - Home, Mailing or Work
  • Address Information 

• Contact Details 
  • Primary or Alternate Contact 
  • Contact Method - Personal or Business
  • Contact Method Type - Phone, Email or Fax 
  • Contact Method Details

• Insurance
  • Primary or Secondary
  • Relationship - Self, Spouse, Child, Other, Unknown 
  • Payer - add payer location 
  • Member ID
  • Group ID
  • Upload Insurance Documents

• Identifiers 
  • Primary 
  • Identifier Type - MRN or Other
  • Value - Identifier number, etc. 
  • Identifier Label - name for this label 

Select Save to close out patient creation
